Outcomes After Intracranial Rescue Stenting for Acute Ischemic Stroke

چکیده

Background In cases of failed recanalization despite modern mechanical thrombectomy (MT) techniques, intracranial rescue stenting (RS) may be beneficial. However, outcomes and complications RS relative to the natural history ongoing emergent large vessel occlusion are unknown. To evaluate whether for after MT achieves superior persistent occlusion. Methods Patients from Stroke Thrombectomy Aneurysm Registry who underwent 2014 2019 were analyzed. For outcome comparisons, patients screened inclusion/exclusion criteria 3 major randomized, controlled trials. Results Over 5 years, 2827 thrombectomy, which 120 required revascularization. resulted in reperfusion (Thrombolysis Cerebral Infarction≥2B) 85.8%. Good 90‐day clinical (modified Rankin scale 0–2) achieved 33.9% patients. Inclusion/exclusion was met 50 MR CLEAN (Multicenter Randomized Clinical Trial Endovascular Treatment Acute Ischemic Netherlands), 64 ESCAPE (Endovascular Small Core Proximal Occlusion Stroke), 45 DAWN (DWI or CTP Assessment With Mismatch Triage Wake‐Up Late Presenting Strokes Undergoing Neurointervention). Of meeting trial criteria, 40.8% cohort modified 0–2 versus 19% medical arm ( P <0.001) 27% 13% =0.04). There no difference =0.15). Symptomatic hemorrhage not significantly increased compared with =0.06), but DAWN. Conclusion This retrospective registry suggests that trial‐eligible yields improved over revascularization, significant increase hemorrhagic events early windows comparable successful at intermediate timeframes.

Intracranial Stenting for Acute Ischemic Stroke

Stroke is the third major cause of death in the US. In the past decade there has been an exponential growth in modalities to treat acute stroke with acute recanalization therapies, including intravenous thrombolysis, intra-arterial chemical thrombolysis and mechanical thrombectomy, thromboaspiration, or angioplasty. While these new modalities of therapy have been promising, there remains a very...

Autopsy Findings After Intracranial Thrombectomy for Acute Ischemic Stroke

Background and Purpose—Endovascular thrombectomy is an increasingly used treatment for arterial occlusion in acute stroke. Various devices (including most extensively the Mechanical Embolus Removal in Cerebral Ischemia [MERCI] Retriever device) have been used for this. Methods—We review the neuropathologic findings in 5 patients (age range, 59 to 87 years) who died acutely or as late as 38 days...

Intracranial Hemorrhage after Endovascular Revascularization for Acute Ischemic Stroke

Objective: Intracranial hemorrhage (ICH) after endovascular revascularization (ER) for acute ischemic stroke is associated with poor outcome. In this study, we examined the risk factors for postoperative ICH in patients who underwent ER in our hospital. Methods: We investigated the incidence/type of postoperative ICH and risk factors in 157 patients who underwent ER in our hospital from July 20...

dysphagia in acute ischemic stroke

Introduction: Swallowing dysfunction complicates acute strokes in 25-32% of cases and directly affects patientschr('39') prognosis and recovery. Dysphagia complicates the course of acute strokes through its potential of the development of chest infection, nutritional problems, and dehydration.  Dysphagia is also an independent predictor of respiratory morbidity and mortality in acute stroke. In...

Angioplasty and stenting for mechanical thrombectomy in acute ischemic stroke.

A large number of patients presenting with acute ischemic stroke have large artery intracranial occlusions, and timely recanalization of these occlusions often leads to improved neurologic outcome. Starting with the widespread use of IV tissue plasminogen activator, a wide variety of pharmacologic and mechanical methods have been introduced to improve vessel recanalization and clinical outcome ...
